The good news is that the state of Oregon does not require a referral for physical therapy.

HOWEVER, some insurance companies DO require a referral/prescription before providing any coverage. It’s always a good idea to call the phone number on your insurance card for details, or give us a call and we can help!

If have a prescription/referral from your medical provider, we’ll work closely with them and provide them with updates on your progress.

We’re happy to help you with referral questions – just call us at 541-383-8179 (Marshall Ave Clinic) or 541-382-2070 (Wyatt Ct Clinic). Either number is fine!

We work with most major commercial health insurances. We also bill Medicare, as well as most major Medicare Advantage plans including PacificSource Medicare, Moda Medicare, Providence Medicare, and others.

Our front office staff knows a thing or two about insurance and are here to help you understand your insurance benefits. We’ll collect your billing information before your first visit to verify all benefits and insurance coverage.

Remember however, ultimately your bill is your responsibility regardless of insurance coverage. It’s always a good idea to contact the customer service phone number on your insurance card to learn about your individual coverage and how it applies to physical therapy.

Copays and Coinsurances are collected at each appointment. If you have an unmet deductible, Therapy Works Physical Therapy requests a $50.00 deposit toward that deductible at the time of service.

Yes, we bill federal Medicare. Medicare covers 80% of the cost of your physical therapy after your yearly deductible has been met. We are also contracted with most major Medicare Advantage plans including PacificSource Medicare, Moda Medicare, Providence Medicare, and others.

If you have a secondary insurance, we transfer the remaining balance to your secondary insurance.

If you don’t have a secondary insurance or your secondary insurance doesn’t cover all of your therapy, you’ll be responsible for the balance.

We offer a Self Pay rate for uninsured patients. Our front office staff is happy to provide more information for you.

At this initial visit, you will discuss your current condition with your physical therapist and identify your personal goals for improvement. Your PT is committed to truly listening to you and empowering you with the tools and knowledge for you to improve and to move forward!

Your therapist will conduct a comprehensive evaluation to determine your general mobility, range of motion, flexibility, and functional limitations. During the evaluation, your PT will be assessing you in order to create your personal individualized treatment plan. This treatment plan is designed to address your specific situation and goals for recovery and improvement.

You’ll generally be scheduled for follow up visits 1-2 times per week, as needed, in order to reach your treatment goals. Our friendly Patient Care Coordinators will help schedule follow-up appointments that best meet your schedule and Plan of Care.

We now offer paperless registration! You will receive a link via email or text message before your appointment. Just click this link and you can fill out intake forms online.

Or, you may print the Patient Registration Forms from this website to complete at your convenience and bring them with you.

(If you prefer to do your intake forms in person, please arrive 15 minutes prior to your Initial Evaluation to allow enough time for paperwork.)

Wear loose comfortable clothing, and please have:

  • The prescription or referral from your physician if you have one.
  • Your current health insurance card and photo ID.
  • If you are being seen for a Worker’s Compensation or Motor Vehicle Accident injury, please bring claim information
  • We collect payments at the time of your appointment and accept Visa, Mastercard, FSA/HSA cards, checks, and cash
